So that you don't get off track we have created a yearly portfolio for you to handle your own evaluations and logs. We have included information that we feel is important and helpful - use it. The portfolio will be amended each year.

Each year there are many requirements for documentation of the educational experiences you have in residency. Because this is much like your future requirements to maintain certification and licensure, this will be your ongoing responsibility. Please be responsible and remember that it is easier to complete these documents over time rather than saving all of them for the end of the year.

Please also remember that you need to attend at least 50% of conferences, dictate discharged patients' charts, and complete rotation evaluations on New Innovations in a timely manner. And not to be forgotten, you need to LOG LOG LOG…
